The McIntosh And Fraser Dental Orthodontic Scholarships

SCHOLARSHIP VALUE

The McIntosh and Fraser Dental Group Orthodontic Scholarships are two prizes valued at up to NZD$10,000 to be used only towards orthodontic treatment to be completed at Fraser Dental or McIntosh Dental.

The (up to) $10,000 prize for the Orthodontic Scholarship is not transferable to any other individual, it is not redeemable for cash, unused portions of the prize may not be transformed into cash, and it may only be used for orthodontic treatment received at Fraser Dental or McIntosh Dental.

The prize is valid for 36 months after the date of award and the orthodontic treatment must be completed within this time frame. If the treatment has not been completed within 36 months, the McIntosh and Fraser Dental Group reserves the right to charge private fees for the remainder of treatment. The McIntosh and Fraser Dental Group also reserves the right to end treatment early with the consent of the patient if the treatment is not progressing according to the agreed treatment plan.

SELECTION CRITERIA

Currently enrolled students in Years 7 – 13 of the following schools may be nominated for the Scholarship:

  • ACG Sunderland College
  • Henderson High School
  • Liston College
  • Massey High School
  • Rutherford College
  • St Dominic’s College (Henderson)
  • Waitakere College
  • Henderson Intermediate
  • Te Atatu Intermediate
  • Rangeview Intermediate
  • Bruce McLaren Intermediate
  • Middle School West Auckland
  • Hobsonville School
  • Whenuapai School
  • Hobsonville Point Primary School
  • Hobsonville Point Secondary School
  • Marina View School
  • West Harbour School
  • Scott Point Primary School

For the elimination of doubt, the student must be enrolled and attending their school on the date of the Scholarship being awarded. If a student leaves their school before the date of the Scholarship being awarded, they will forfeit their entry in the Scholarship.

Anyone may nominate a deserving student for the Scholarship.

Applications will be received between 1st July to 31st July. Applications received after this date will not be considered.

Applications must be made via the online nomination form on the Fraser Dental (or McIntosh Dental) website.

Incomplete applications will not be accepted, and Fraser Dental reserves the right to contact the person nominating to clarify details in the application.

Nominees will be considered on achievement in Arts, Sports, Academics, Cultural activities, or contributions to the Community.

In August, all applications will be considered by the Orthodontic Scholarship Committee. Finalists will be chosen and they will be invited to attend Fraser Dental or McIntosh Dental for a complimentary orthodontic consultation. The appointment will clinically assess the finalist’s orthodontic condition and confirm their candidacy for the Scholarship.

The announcement of the two Scholarship recipients will be made after all orthodontic consultations with the finalists have been completed.

The unsuccessful finalists will be presented with an oral health gift bag to thank them for their involvement in the Scholarship.

The Scholarship Committee’s decision is final and no correspondence will be entered into.

ORTHODONTIC TREATMENT

The Scholarship recipients will each be awarded a prize up to the value of $10,000 to be used towards a course of Orthodontic treatment at Fraser Dental or McIntosh Dental. Treatment will only proceed with the full cooperation and informed consent of the patient, and their parents/guardians.

ORTHODONTIC SCHOLARSHIP WINNER OBLIGATIONS

The scholarship recipient consents to have their photo taken and used on the Fraser Dental or McIntosh Dental websites, blogs, newsletters, Facebook and Instagram pages. The Scholarship recipient will be asked to allow their clinical photos to be used for the purposes of patient education (e.g. before and after treatment).

From time to time during treatment, the Scholarship recipient may be asked for treatment progress photos and/or their experiences to be shared with various media for educational or promotional purposes.
